Delete a Pending Intake Form (Fusion)

  Permissions: You can only manage intake forms for patients you have access to.

If you've sent a pending intake form to a portal user, but no longer need them to complete it, you can delete that pending form.

Before You Begin

Deleting a pending intake form will completely remove it, as if you had never sent it to the portal user. If they have submitted any information on the form, you might want to save a copy of it before deleting it.

Delete a Pending Intake Form

1. Go to to Schedule → Pending Intake Forms.
2. Click the form you want to delete.
3. Click the Delete button.
4. Click the Delete Intake Form button.
