Permissions: You need the Admin permission to add an intake form to the clinic.
Add an intake form to your clinic to let users send the form to patient contacts through the patient portal.
Before You Begin
Consider uploading a fillable PDF so your clients can fill in the form's information without having to print it out.
Add an Intake Form
- Go to Admin → Advanced Setup → Intake Forms.
- In the lower right, click the + Intake Form button.
- Fill in the information for this intake form.
- Click the Save Intake Form button.
About the Fields
|Name||The name of the intake form, which will be visible to portal users.|
|Description||An internal description for the intake form. This field is only visible to your clinic's Fusion users.|
|File||Select the PDF file to use as the intake form that portal users will fill out. This file must be 20 MB or less.|
