Add tables to Custom Intake Forms to group multiple fields of related information by displaying the fields as side-by-side columns. You have the option to include text, radio buttons, checkboxes, dropdowns, dates and pictures in your tables. Use Row Labels to control or limit the number or answers or responses available to the person filling out the form, or create a table without Row Labels to give the person filling out the form the option to enter as many answers or responses as they want.

Create a Table with Row Labels
Use Row Labels to control how many rows display in your table. The person filling out the custom intake form is unable to add additional rows to a table with Row Labels. You can use this to limit the number of options for answers or responses to the person filling out the form.
- Click the Table form item from the list on the left and drag it into the form.
- Select Row Labels from the Type dropdown list in the Add Column window.
- Click the Save Column button.
- Enter a Column Label.
- Enter Row Labels. Click +Row Label to add additional Row Labels.
- Click the Save Row Labels button. The Table displays with the Row Labels you defined.
- Hover over the Column Label and click the Gear icon to Edit or Delete a column, or to add another column to the table.
- Click +Column.
- Select your column Type from the dropdown list in the Add Column window. (The example below uses the Short Text Field for Column Type.)
- Click the Save Column button.
- Enter and Save the Column Label and other column information.
- The second Column displays next to the Row Labels. Hover over the column names and click the Gear icon to add more columns, or to adjust column placement within the table.

Create a Table Without Row Labels
If your table does not contain Row Labels, the person filling out the custom intake form has the option to add additional rows under the columns you create using an +Row option that appears on the customer-facing version of the form. This allows the person filling out the form to add rows for additional responses and answers.
- Click the Table form item from the list on the left and drag it into the form.
- Select your column Type from the dropdown list in the Add Column window. (The example below uses the Dropdown for the first Column Type and Short Text Field for the second Column Type.)
- Click the Save Column button.
- Enter and Save the Column Label and other column information.
- Hover over the column names and click the Gear icon to add more columns, or to adjust column placement within the table.
- The Columns display side by side in the table.
